Overview

The Cigna Group is a global health company operating through Evernorth Health Services, which provides pharmacy, specialty and care services, and Cigna Healthcare, which offers medical coverage and related solutions to employers, individuals and families in the U.S. and international markets.[1] Its stated mission is to improve the health and vitality of the people it serves through personalized, transparent and affordable health care.[7] At year-end 2025, the company reported approximately 67,700 colleagues, more than 185 million customer relationships and operations in over 30 markets and jurisdictions; its 2025 revenue was $274.9 billion.[8][9] Cigna was formed in 1982 through the combination of INA and Connecticut General.[10] [1]

Products & services

Cigna Group’s pharmacy-benefit products and services include drug-claim adjudication, retail-pharmacy network administration, benefit-design consulting, drug-utilization review, formulary management and home delivery. Express Scripts Pharmacy offers standard shipping, usually in 90-day supplies, eligible automatic refills, and access to care advocates and pharmacists. Evernorth EnGuide Pharmacy provides clinical support for customers using GLP-1 medicines. The 2025 annual report lists 11 licensed Express Scripts pharmacies and two EnGuide pharmacies.[11][12]

Accredo provides specialty-pharmacy services for people using medicines for complex or rare conditions, including clinical monitoring, patient training, in-home nursing, reimbursement support and condition-focused Therapeutic Resource Centers. CuraScript SD distributes injectable and infusible medicines and other specialty drugs and supplies to clinics and hospitals for office administration; its disclosed capabilities include four distribution centers, mostly overnight U.S. shipping and third-party logistics for some pharmaceutical companies.[13][14]

Care offerings include virtual primary and urgent care, behavioral health and dermatology through MDLIVE, as well as onsite low-acuity and primary-care services. Evernorth’s named offerings also include EviCore’s evidence-based medical-care solutions. Cigna Healthcare employer plans include medical options such as Open Access Plus, PPO, LocalPlus, SureFit and HMO, alongside pharmacy, dental, vision, behavioral-health, supplemental-health, wellness and spending-account offerings; availability varies by plan and location.[15]

For the six months ended June 30, 2026, Evernorth reported adjusted revenue of $119.910 billion: $67.498 billion from Pharmacy Benefit Services and $52.412 billion from Specialty and Care Services. Cigna Healthcare reported adjusted revenue of $23.205 billion. These are segment-level figures, not product-level revenue shares.[21][22] [11]

Geographic presence

At year-end 2025, The Cigna Group had approximately 67,700 employees worldwide, about 88% of whom were based in the United States. Its real estate footprint comprised approximately 6.3 million square feet in the United States—across 42 states, Washington, D.C., and the U.S. Virgin Islands—and 1.8 million square feet in 23 countries internationally.[23][24]

The company reports a U.S. health-care business as well as International Health operations serving international markets, globally mobile people, and multinational employers. Its official overview describes sales capabilities in more than 30 markets and jurisdictions. These measures describe operational and sales reach, not the distribution of revenue: the reviewed disclosures do not provide revenue shares by country or region, or employee counts by country.[6] [23]

Business segments

For FY2025, The Cigna Group reported two segments: Evernorth Health Services and Cigna Healthcare. Evernorth includes Pharmacy Benefit Services and Specialty and Care Services, offering independent and coordinated health solutions. Cigna Healthcare provides medical and coordinated health-benefit solutions through its U.S. Healthcare and International Health businesses.[2]

Using reported total revenue of $274.9 billion as the denominator, Evernorth contributed approximately 85.5% ($235.0 billion) and Cigna Healthcare approximately 17.2% ($47.4 billion). These reported amounts include intersegment revenue and net investment income. Other Operations contributed about 0.2% ($0.7 billion); Corporate and Eliminations were a negative 3.0% (−$8.1 billion), reconciling segment amounts to consolidated total revenue.[27][28] [2]

Company history

The Cigna Group traces its origins to two predecessor insurers: the Insurance Company of North America, founded in Philadelphia in 1792, and Connecticut General Life Insurance Company, incorporated in 1865. The companies merged in 1982 to form CIGNA.[30][31]

CIGNA expanded into managed care and senior-focused services, completing its acquisition of HealthSpring in 2012. In 2018 it combined with Express Scripts, adding a pharmacy-benefit business; in 2020 it introduced Evernorth as the brand for its health-services portfolio and sold its group life, accident and disability business.[32] In 2023, Cigna Corporation adopted The Cigna Group as its holding-company name. The company completed the sale of its Medicare businesses and CareAllies in 2025.[36][37] In 2026, Brian Evanko succeeded David Cordani as CEO, with Cordani becoming executive chair.[38] [39]
